Telehealth informed consent

You will be asked to read and accept this consent on the secure intake before a physician reviews your assessment. It is published here so you can read it in advance and return to it at any time.

What telehealth means here

Telehealth is the delivery of health care using electronic communications when you and your physician are not in the same place. Menova uses asynchronous (store‑and‑forward) telehealth: you complete a written health assessment, a physician licensed in your state reviews it at a later time, and you communicate through secure messaging. There is no scheduled video or phone visit unless your physician determines one is needed or your state requires it.

How your care works

  1. You complete an assessment about your age, cycle, symptoms, medical and family history, medications, and goals, and confirm your identity.
  2. A physician licensed in your state reviews your answers. They may message you with follow‑up questions or request labs, records, or a synchronous (live) visit.
  3. If hormone therapy is appropriate, the physician issues a prescription that is sent to the dispensing pharmacy. If it is not appropriate, the physician will tell you and may suggest alternatives or in‑person care. You are not charged if you are not prescribed.
  4. Your physician remains available by secure message, checks in with you after about four weeks, and may adjust or discontinue treatment based on your response.

Your rights

Privacy and records

Your assessment, messages, and prescriptions become part of your medical record, which is maintained by DrTelx in accordance with HIPAA and state law. Information will not be shared without your authorization except as permitted by law and described in the Notice of Privacy Practices.

Emergencies

If you experience a medical emergency, such as chest pain, sudden shortness of breath, severe headache, vision or speech changes, or leg swelling and pain, call 911 or go to the nearest emergency department. Do not use messaging for urgent needs.

Licensing and complaints

Your physician is licensed by the medical board of the state in which you are located at the time of care. You may contact that board with questions or complaints; a list of state boards is at fsmb.org. By checking the consent box on the intake you confirm that you have read and understood this document, that your questions have been answered, that you are located in the state you selected, that you are 18 or older, and that you consent to receive care through telehealth as described.
